Shanghai, China — A recent study reveals that the majority of liver cancer cases around the world could be prevented through lifestyle modifications and improved public health strategies. The findings underscore the significant impact of obesity, alcohol consumption, and viral hepatitis on liver cancer rates.
The Lancet Commission on liver cancer indicates that addressing these factors could lead to a notable reduction in new cases. Specifically, the report suggests that enhancing vaccination rates against hepatitis and implementing strategies to reduce alcohol intake and manage fatty liver disease could prevent between 9 million and 17 million new cases by 2050.
Professor Jian Zhou from Fudan University, who spearheaded the research, highlights that liver cancer poses one of the most daunting challenges in oncology today, with five-year survival rates varying between 5% and 30%. Without immediate intervention, experts predict that cases and mortality numbers will nearly double over the next 25 years.
Currently, liver cancer ranks as the sixth most prevalent cancer globally and is the third leading cause of cancer-related deaths. Projections show that fatalities could rise from 760,000 in 2022 to approximately 1.37 million by 2050. The report anticipates nearly doubling the incidence of new liver cancer cases, expected to reach about 1.52 million, primarily driven by an aging population and demographic shifts, especially in Africa and China.
One of the escalating contributors to liver cancer is fatty liver disease, closely linked to rising obesity rates. An estimated one-third of the global population suffers from metabolic dysfunction-associated steatotic liver disease (MASLD), which, if managed through proper diet, exercise, and weight control, could be mitigated.
Moreover, the report notes that while only a minority of those with MASLD develop its more severe form, metabolic dysfunction-associated steatohepatitis (MASH), this group is increasingly contributing to liver cancer statistics. The percentage of liver cancer cases attributed to MASH is projected to increase from 8% in 2022 to 11% by 2050.
Alcohol consumption is also on the rise as a cause of liver cancer, with projections indicating an increase in related cases from 19% to 21% over the same period. Conversely, the rate of liver cancer cases tied to hepatitis B is expected to decline, while cases associated with hepatitis C are also projected to decrease.
Professor Hashem B. El-Serag from Baylor College of Medicine highlights the emerging risk of obesity on liver health, stressing that this trend complicates the traditional understanding of liver cancer’s primary causes, previously dominated by viral hepatitis and alcohol abuse.
The commission emphasizes policy recommendations aimed at curbing the rise of liver cancer. It suggests that governments should prioritize vaccinations for hepatitis B, establish minimum pricing on alcohol, institute sugar taxes, and promote early detection initiatives while enhancing palliative care for patients.
Professor Valérie Paradis of Beaujon Hospital in France underscores the urgency of raising public awareness regarding liver cancer and its preventable aspects. With well-defined risk factors, she believes that targeted prevention strategies could significantly lower case numbers and improve the overall quality of life for those affected by liver cancer.
Dr. Matt Hoare, an associate professor at the University of Cambridge, notes that liver cancer differs from many other cancers in that its death rate continues to rise, attributing this trend to various regional causes. Drawing from Japan’s success in reducing liver cancer fatalities through proactive public health policies and early detection, he emphasizes the importance of ongoing efforts to identify individuals at risk for developing liver disease through advanced testing methods.
